NP or PA - Pain Management
UMMARY/PRIMARY FUNCTION:
The successful candidate for this role will be a clinical expert in patient care and provide a high standard of care to patients needing pain management and/or palliative care following an established protocol with a physician(s). Position provides direct patient care and/or consultation in collaboration with physicians and other members of the interdisciplinary team, focusing on patients with acute and chronic pain
PRINCIPAL DUTIES AND
Responsibilities:
- Perform patient assessments including a medical history and physical exam and in collaboration with the physician develops an appropriate plan of care. This includes but is not limited to medical diagnosis, symptom management, diagnostic testing, medical therapy, and teaching.
- In collaboration with a physician, diagnose and treat a wide range of pain complaints
- Coordinate with multidisciplinary care teams, social workers, case managers, patients and families to create an individualized pain plan for each patient.
- Efficiently monitor the patient's progress through follow-up visits. Perform focused evaluation of patients. Review the medical record including physician/consultation notes, lab and diagnostic tests results. Formulate plan of care based on above data
- Participate in the collection of research data, quality improvement, discharge planning and coordination of patient care for consulted patients.
- Integrate current knowledge of professional issues, trends in health care and technological advances into scope of practice.
- Perform other related duties as required
EDUCATION AND EXPERIENCE REQUIREMENTS:
- Nurse Practitioner requires a Masters' degree in Advance Practice Nursing
- Physician Assistant must be a graduate of an ARC-PA accredited physician assistant program and have NCCPA Certification
- Candidate must be appropriately licensed as a NP or PA or be eligible for licensure in Delaware and Pennsylvania as appropriate
- Experience in clinical pain management as an acute care, emergency room, outpatient, or intensive care NP or PA preferred.
- Palliative Care experience preferred
- Prescription Authority required
- DEA Registration required
