Job Title:
Director of Wellness - RN/LPN This candidate is responsible for the assessment of resident health status for both new and existing residents and the subsequent development of the care of the service plan.
Under the direction of the Executive Director, this position is responsible for effective communication with direct-care staff to ensure the plan of care is being delivered effectively.
This candidate provides ongoing audit and reviews with staff to ensure compliance with State Regulations and policies.

Responsibilities:
Communicate with the Executive Director on resident needs on an as needed basis to ensure consistent quality service.
Develops a plan of care consistent with the needs of the individual resident upon admission, quarterly, and as needed due to changes in condition.
Participate in providing hands-on care and specific nursing tasks as required and in support of the resident's plan of care.
Provide clinical support and training of direct-care staff in requirements of continuing education Transcribe new and existing physician orders to ensure medications and care delivery is in agreement with current orders.
Assists in developing methods for communication with other resident services to ensure the continuity of resident care.
Participates in facility surveys (inspections) made by authorized government agencies as directed by the DHS or the Executive Director.
Maintain proper licenses required by state and local licensing agencies.
Plans and coordinates on-going in-service training for continuing education with co-workers.
Shows initiative, performs beyond job responsibilities, and involves themselves in a teamwork approach.
Communicate effectively with management, co-workers, residents, families, visitors, and other disciplines.
Adheres to procedures for fire, life safety, disaster, security, worker safety and other emergency procedures.
Maintain resident confidentiality and adhere to HIPPA policies.
Seek out and perform other duties as assigned or needed.

Qualifications:
Must have compassion for and desire to work alongside our aging population! High School Diploma or GED Valid State Nursing License (RN or LPN).
Certifications as required by the State.
Must have 1 year experience as a nurse or caregiver preferably in long-term care preferred.
Supervisory experience of 6 months or more in a health-related field.
Must be able to read physician orders and match to Medication Administration records for residents.
Understand medical terminology to pass medications and communicate with medical professional colleagues.
Effectively communicate with families, residents, and co-workers.
Experience with Microsoft Word, Microsoft Excel, and Understanding State Regulations and Human Resources Rules.
Must be able to pass a criminal background check and drug test.
